Oooiii everyone....! I need your help!

I'm looking for video converter software that can convert DVD to h.264 formats(mkv).

The software must have chapter extraction feature and subtitle extraction feature, also the output is single file (.mkv).

Anyone know?

Thanks for your help....!

Added after 2 minutes:

I know the similiar software, it's Nero Recode. But Nero Recode convert DVD to h.263 formats(mp4).

Ummm, have you heard of HandBrake?  I don't know if it does everything you're talking about, but it's great for ripping DVDs and does encode H.264 and allow you to use MKV as a container.  It's a great program, and it's FOSS - awesome, huh?

Looks like this is the one that i am always looking for...!

How did you find it? The name is somewhat strange and irrelevant, it doesn't look like video converter names at all! That's why the search engine can't find it..... ugh...

But the feature is great...

By the way, thanks Ian!

You're very welcome.  I came across the nifty little utility when looking for a program that could rip DVDs into video for my mp4 player.  The weird thing is it was originally designed for BeOS o__O, really, it's on wikipedia.
